Create control like pop-up with text field and check boxes

Hi,

How should I create control with text boxes and list with check boxes?

I dont know what to use matrix layout ,grid ,form ...

I need some text box and under it there is list of values and for every value there is

a chekbox in the left side,there is example how to create it right ?
